If you wish to register for Online Access, please print and complete the application form below or collect one from reception. In either case, please bring the completed form to reception together with identification (photo ID e.g. passport/driving licence and proof of residence e.g. utility bill).
Please note that due to GDPR, it is no longer possible for patients to share the same email addresses and all applications will need to include a unique email address that is used by the patient only.
Once you are registered you will be able to use the service to:
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
- Review your medications and known allergies
